Referrals, Authorizations, & Payment Options
A prescription for therapy from your referring doctor is required before your first appointment. Our administrative staff will assist you in obtaining an authorization from your insurance company. Therefore, you will know how many visits will be covered as well as your financial obligations.
Our clinic accepts many PPO & HMO insurances, Medicare, Tricare as well as third party contracts for workers' compensation cases. All insurance co-payments are due at the time of your visit. We accept cash, checks, Visa, MasterCard, American Express, and Discover Card for your convenience. We will help in any way we can to assist you in handling claims. If your insurance policy has a high deductible, please let us know so we may discuss your options. We offer discounted cash rate for private pay.
